My internal medicine rotation has been a great introduction into understanding the complexities of medicine. Throughout this rotation, I’ve come to appreciate the importance of mastering procedures such as blood draws, NG tube placements, and foley catheters. I found myself challenged when trying to master these skills and hop that through more practice I will feel more comfortable doing them in the future. I found myself gaining many opportunities to do procedures and even sought out my own. One particular day I really liked was seeing the wound care team and being able to follow their care for the day. I was able to debride wounds and learned about the various ways to pack and dress wounds properly. What I found to be really different and interesting about this rotation , is that there was a lot more time spent managing patients care and following their care over times compared to all my previous rotations. It allowed us to have the time to focus on their acute small changes instead of only the life threatening ones like in the emergency room. One patient in particular that I will always remember is this patient with Glioblastoma Multiforme who was in hospice and was not wanting to continue treatment. However, he had a new tumor that was impacting his personality and his ability to function acutely. The patient was therefore given a new option to continue care or to continue hospice. The palliative team was consulted, and the patient was given the option of what to do. The patient ended up choosing to continue hospice, but what I found interesting was seeing the different specialties that were called in order to make that decision. Some of which were palliative care, psychiatry, and neurology. It taught me about the interconnectedness of healthcare and how each system might work alone but yet they all function together in a way as well, just like different hospital specialists. In summary, my internal medicine rotation has been a wonderful learning opportunity, in which I enriched my understanding of medicine, gained new procedure skills, and taught myself more about the need for patient advocacy and multidisciplinary approaches to patient care. I had a great experience and hope to continue to improve my internal medicine procedure skills on other rotations.
